Disseminated intravascular coagulation is a pathologic syndrome with different medical disorders. Diagnosis and treatment of this syndrome is one of the difficult managements in medical science. Thromboelastogram is the only guide for early diagnosis and precise management of this syndrome. We describe a patient who developed disseminated intravascular coagulation due to endocarditis and spleen abscess. She was diagnosed by thromboelastography and treated successfully.

While a "closed discovery system" is where information is searched from A B and C B. Although several examples have since been reported, the actual use of knowledge discovery systems has been difficult. There would be at least five main reasons. The first of these is 1 ; vocabulary problems: the recognition of multiwords, synonyms, hyponyms, and hypernyms cannot be resolved without a sufficiently large thesaurus. The second is the 2 ; low frequency term concept problem: the recognition of relationships between low frequency terms is quite difficult, if they only use statistical probabilistic sorting for co-occurrence data. The third is the 3 ; candidate B-concept intermediate term ; selection problem: in some cases, too many or too few candidate B-concepts occur. The fourth is 4 ; insufficient intermediate steps: in some cases, only one intermediate step is insufficient to connect hidden relationships. The last is 5 ; background knowledge: in most cases, the researcher cannot determine whether the B-concept is a meaningful candidate without adequate background knowledge. Most systems have introduced various ingenuities to overcome these problems. For example, the use of bigrams and trigrams extracted from text Gordon & Lindsay, 1996, 1999 ; , the use of MetaMap Aronson & Rindflesch, 1997 ; to recognize Unified Medical Language System UMLS ; concepts in the text Weeber et al. 2000, 2001 ; , and the use of MeSH Medical Subject Headings, the meta-data, attached to each abstract ; terms Srinivasan & Libbus, 2003; Srinivasan, 2004 ; have been reported for term concept recognition. A semantic filter for the UMLS has been reported Srinivasan, & Libbus, 2004; Weeber et al., 2000, 2001; Srinivasan, 2004 ; to effectively select candidate B-concepts and C-concepts. We developed the "BioTermNet" as a knowledge discovery system for the biomedical domain and adopted the following methods to resolve these problems as far as possible. It included 1 ; the use of dictionaries developed in our laboratories and publicly available thesauruses. It also made 2 ; combined use of a statistical approach and syntactic analysis. As syntactic analysis data, relationships between protein-protein gene functions extracted by syntactic analysis PRIME data, Koike & Takagi, 2004b ; were used. Further, it incorporated 3 ; the addition of a semantic type to enable intermediate candidate concepts to be focused on, similar to previous reports Weeber et al., 2001; Srinivasan, 2004 ; . It also enabled 4 ; multiple intermediate steps AB1B2C model, AB1B2B3C model, . ; to be considered. Finally, it permitted 5 ; evidential sentences to be presented for syntactic analysis PRIME data ; , and evidential abstracts to be presented for the statistical approach so that a user could check whether this relationship was appropriate or not and know what kind of relationship was involved such as a regulatory relationship. BioTermNet's purpose is not only to present hidden relationships but also to present information to enable users to confirm or investigate the relationship actually described in the text. Both the "open discovery process" and the "closed discovery process" can be used in our system, because alfuzosin vs tamsulosin.

The National Kidney Foundation's Kidney Disease Outcomes Quality Initiative K-DOQI ; guidelines offer basic principles for managing lipid abnormalities in CKD patients, including those on dialysis. Experts in the field base these guidelines on their review of the literature. These guidelines differ somewhat from those for the general population proposed by the NCEP ATP-III. K-DOQI recommends that stage 5 CKD patients be evaluated upon presentation when the patient is stable ; , at 23 months after a change in treatment, and at least annually thereafter. Patients on HD should have their blood drawn either on a nondialysis day or before the dialysis procedure since HD may affect lipid levels.19 Unlike the NCEP ATP-III, the K-DOQI target for LDL-C in CKD patients is 100 mg dL since they are at high risk for cardiovascular events.19 Drug therapy is recommended for patients with an LDL of 100-129 mg dL after 3 months of therapeutic lifestyle adjustments, whereas it is considered optional in the NCEP ATP-III.19 The initial drug of choice for treatment of an elevated LDL-C by K-DOQI guidelines is a statin.19.

Despite our frequent use of sedation we do not yet fully understand all the aspects of utilizing sedation adequately, safely and cost effectively. There is no general agreement on which is the most appropriate method for assessing sedation. The best approach is to use some tool for assessment consistently. We must be aware that much of what is known of the drugs we use for sedation was gleaned from healthy volunteers and that our patients will not react the same. With prolonged use many of our sedative agents will exhibit some degree of tolerance and will not emerge as rapidly as expected. As we continue to use prolonged courses of sedatives, we see the emergence of withdrawal becoming more common. Many drug interactions have not been adequately described and more seem to be discovered daily. Our approach to sedation would include setting a treatment goal, deciding how we will assess it, and selecting the most appropriate drug for a patient's situation even if this requires the use of more than one agent or infusion. At some pre-determined time e.g. daily ; , the goal should be reevaluated and all patients should be watched for the emergence of withdrawal and fenofibrate.

Assignment is unreadable until after allocation. Other robust methods considered adequate to prevent foreknowledge of the allocation sequence to clinicians were also considered as fulfilling this criterion. Any predictable or decipherable sequence was considered inadequate. In the majority of studies 78 ; , it was unclear whether the method used in the study was adequate, as the method used was not reported. However, 37 studies used methods that were considered adequate. 6. Were the outcome assessors blinded to the treatment allocation? Outcome assessors may have an opinion about the efficacy of the treatment under investigation. This may influence the reporting of the outcome data if the treatment allocation is not blinded, thereby distorting study findings. In addition, if outcome assessors are aware of which treatment patients are receiving, this may influence their interpretation of marginal findings or cause them to provide differential encouragement during assessments. Blinding of outcome assessors ensures that they are ignorant of the allocated intervention. This protects against performance and detection bias, and can also contribute to adequate concealment of allocation. The majority of studies 80 ; did not report whether outcome assessors were blinded to treatment allocation. Twenty studies reported outcome assessors to be blinded and 10 reported that they were unblinded. In four studies assessors were blinded to some but not all of the outcomes assessed.4851 7. Were those individuals who administered the intervention blinded to the treatment allocation? Individuals administering the intervention may have an opinion about the efficacy of the treatment under investigation. Again blinding prevents any such individuals from unduly influencing the trial outcomes. Blinding of individuals administering the intervention ensures that they are ignorant of the allocated intervention. This protects against performance and detection bias, and can also contribute to adequate concealment of allocation. The majority of studies 68 ; did not report if those administering the interventions were blinded to treatment allocation. Where reported, administrators were blinded in 31 studies and unblinded in 16 studies.

ABSTRACT: Snakebite is an important medical emergency. Anti-snake venom along with supportive care is the only specific treatment. However, many people put their faith in non-registered medical practitioners. Where medical aid is available, lack of trained health personnel jeopardizes the situation. This retrospective study, the first of its kind, was aimed at studying the pre-hospital treatment as well as the behavior of patients bitten by snakes and referred to the Postgraduate Institute of Medical Education and Research, Chandigarh, India. A total of 88 cases that occurred between January 1997 and December 2001 were studied. Seventy patients received treatment prior to admission the majority was treated by non-registered medical practitioners, registered medical practitioners, and MBBS doctors ; . The various treatment modalities used were: anti-snake venom ASV ; , tourniquet, incision and drainage I&D ; , tetanus toxoid, injections, and tablets. Non-registered medical practitioners still preferred tourniquet and I&D. The patients who were referred within 24 hours stayed less time in the hospital and spent less money on the treatment compared to those who were referred after 24 hours. Non-registered medical practitioners and inadequately trained health staff are often the first contact of snakebite victims. Their traditional and unscientific methods of treatment lead to unnecessary morbidity and increased treatment cost. It is therefore necessary to train these people adequately so that proper treatment can be instituted at the earliest. KEY WORDS: snake envenomation, clinical features, treatment, first aid.
