Various Field Force initiatives were implemented to assist your Company to move forward and attain its objective of achieving global alignment by launching new products, and gaining leadership in the marketplace. The Pharmaceutical Field Force was restructured into three smaller therapy-focused teams, designed in a way to concentrate on target doctors in defined specialties, with products assigned in a manner that would help leverage their franchise and establish Pfizer's leadership in core therapy areas. An immediate offshoot of this transition was an increase in Field Force productivity and the consequent strong growths achieved by key products. Information technology was leveraged through OPTIMA II the online web enabled Field Force reporting and work scheduling system, coupled with OPTILEARN - the e-learning module, to augment the selling capabilities and improve the effectiveness of your Company's sales force. A new software program MEDICO for order processing was implemented at thirty-five CFA locations and three warehouses across the country. Modernization of this Inventory Control System will enable all stakeholders in the sales operation to analyze data and make informed decisions, thus enhancing the efficiency of the entire distribution chain, for example, tagamet ingredients.
Comprises a relatively small but significant percentage of the total cytochrome P450 in the liver. Only 2% to 6% of total liver cytochrome P450 is CYP2D6, but approximately one-quarter of clinically useful medications are metabolized by this enzyme. In particular, many antiarrhythmics and beta-blockers are metabolized by CYP2D6. In addition, CYP2D6 is responsible for the conversion of codeine to morphine, accounting for the majority of its analgesic effects. Unlike other CYP enzymes, there are no known inducers of this activity, except pregnancy. Several medications are known to inhibit CYP2D6, the most potent of which include quinidine, paroxetine Paxil ; , and fluoxetine Prozac ; . Other inhibitors include: sertraline Zoloft ; , thioridazine Mellaril ; , cimetidine Ragamet ; , amiodarone Cordarone ; , diphenhydramine, haloperidol Haldol ; , and ticlopidine Ticlid ; . Of the anti-hiv drugs, only ritonavir is known to inhibit CYP2D6 activity. Genetic factors play a particularly significant role in determining CYP2D6 activity. FORMULARY PRODUCT DESCRIPTIONS To assist in understanding which specific strengths and dosage forms are on the formulary, examples are noted below. The general principles shown in the examples can then usually be extended to other entries in the book. Any exceptions are noted in the drug list. There may also be a statement associated with a drug list that gives additional information about which specific products or dosage forms are on formulary. The brand names shown are for reference only; a different brand or a generic version may be dispensed. Products on formulary include all strengths associated with the dosage form of the cited brand name product. minocycline Minocin Because Minocin is a capsule, all strengths of minocycline capsules: 50, 75, and 100 mg are on formulary. Modified-release or combination products on formulary are defined by the cited brand name product. verapamil ext-rel Calan SR Only Calan SR products including generic versions are on formulary based upon the entry. Products based upon brand name references of other verapamil ext-rel products, e.g., Covera HS, are not included on the formulary unless they are also listed. Extended-release and delayed-release products require their own entry. dextroamphetamine Dexedrine The long-acting product Dexedrine Spansule is not on formulary based upon the Dexedrine entry. glipizide ext-rel Glucotrol XL This entry confirms that the extended-release product is on formulary. Dosage forms on formulary will be consistent with the category and use where listed. neomycin polymyxin B hydrocortisone Cortisporin Since Cortisporin is listed only in the OTIC section, coverage is limited to the otic solution and suspension. From this entry the ophthalmic solution and ophthalmic ointment, and the topical cream cannot be assumed to be on formulary unless there are entries for these products in the OPHTHALMIC and DERMATOLOGY sections of the formulary. Oral liquids and orally disintegrating tablets for products cited as immediate-release or delayed-release enteric-coated ; oral solids are on formulary. Likewise, suppositories are on formulary when they are used as an alternative to the tablet capsule. cimetidine Taamet In addition to the tablets, the oral solution is on formulary. indomethacin Indocin In addition to the capsules, the oral suspension and suppository are on formulary. ondansetron Zofran In addition to the tablets, the oral solution and orally disintegrating tablets are on formulary and temovate.

In conclusion, stroke is now becoming a much more preventable disease. A significant drop in the incidence of stroke in the future should not be far away. Expedite diagnosis and early implementation of preventive measures would help to translate the observed benefit from trials to clinical practice. Introduction Doxycyclin is a broad-spectrum antibiotic, with activity against a wide range of gram positive and gram-negative organisms [1]. It is the drug of choice in the treatment of Lyme disease, brucellosis and several rickettsial infections. It is also extensively used for the treatment of juvenile periodontitis [1]. The formation of periodontal pockets has been the result of a localized pathogenic bacterial infection below the gum line [2]. Periodontal pockets are easily accessible from the oral cavity and are therefore a convenient site for treatment by a localized drug delivery system. The particulate based systems for the delivery of drugs are claimed to have enhanced bioavailability, predictable therapeutic response, greater efficacy and safety and controlled and prolonged drug release profile [3, 4]. They are especially important in the case of poor drug distribution at the site of infection due to limited blood circulation to the surrounding skeletal tissue [5]. Various particulate systems containing drugs distributed within a and tetracycline.

Renal involvement in parasitic infections is polymorphic, ranging from direct invasion to various types of glomerulonephritis. Acute interstitial nephritis as a form of hypersensitivity reaction is an uncommon manifestation in the setting of human parasitosis [7]. Acute interstitial nephritis is a heterogeneous disorder not only in aetiology, but also in presentation, laboratory findings and outcome. We present a case of acute interstitial nephritis in association with A.lumbricoides infection of a 48-yearold patient. A combination of antibiotic and steroid therapy led to eradication of the helminths and to full recovery of renal function. The aetiology of interstitial nephritis remains unclear in many patients, particularly in those taking potentially offending drugs. In our case, the patient was not taking any medication and denied taking any drugs, including non-steroidal anti-inflammatory drugs NSAIDs ; , prior to the current hospital stay. In vivo as well as in vitro bleeding time assessed prior to percutaneous renal biopsy had been in the normal range, supporting the notion that at least NSAIDs had not been taken by our patient. Besides invasive ascariasis of the kidney [8] and acute renal failure in the setting of A.lumbricoidesinduced acute pancreatitis [9], to date only three cases of acute interstitial nephritis in A.lumbricoides infection have been described in the literature [10, 11]. In one patient, acute interstitial nephritis was accompanied by pulmonary and hepatic involvement, and in one case by pulmonary infiltrates and generalized lymph node swelling. In the third case reported, no organ manifestation apart from acute interstitial nephritis was observed, as in the case presented here. We excluded pulmonary involvement by X-ray and sputum examination which showed no eosinophils or CharcotLeyden crystals. In none of the described patients did eradication of A.lumbricoides alone lead to recovery of renal function. In the first two cases described, acute interstitial nephritis progressed to acute renal failure leading to death in the pre-haemodialysis era. In the third case dating from 1995, acute interstitial nephritis progressed and haemodialysis was required as a supportive treatment because of transient uraemia. Similar to the clinical course of our patient, in the latter case introduction of steroid therapy led to full recovery of renal function. Infection with A.lumbricoides and presentation of its antigens has been demonstrated to activate granulocytes and lymphocytes and leads to an induction of various cytokines, such as interleukin-4 and interleukin-5 [12]. These mechanisms also play a major role in the pathogenesis of antigen-mediated acute interstitial nephritis and may therefore present a common pathway in these cases [4]. Moreover, it has been described that eosinophils and eosinophil granule proteins are elevated in the urine of A.lumbricoides-infected patients even in the absence of renal involvement [13]. Introductions. This involved many new antibiotic drugs, hydrocortisone and several other cortocoids, the thiazide diuretic and beta blocker drugs for hypertension, new classes of tranquilizers and anti-depressants, and the initial birth control drugs. However, by the early 1970s, the industry was experiencing diminishing returns in many of the drug classes that had seen major advances in the 1950s and 1960s. A number of hypotheses were investigated, including the effects of more stringent FDA regulations, diminishing technological opportunities and increased product liability. Some scholars saw the industry entering a prolonged period of technological maturity.[26] Finding new drugs that were advances over established drugs had clearly become increasingly costly and more problematic by the early 1970s. Many of the leading firms began to focus their R&D activities on new therapeutic targets and approaches. One important concept that took root during this period was the "rational drug-design" approach to R&D. This involved the use of x-ray crystallography and other techniques to design specific compounds that could block particular receptor sites and thereby create desired therapeutic responses. The primary approach to discovering new drug therapies prior to this time involved the random screening of compounds against a small number of known targets. An important milestone for the industry occurred in 1978 with the introduction of Tagameet cimetidine ; by SmithKline. This drug was not only a significant advance in the treatment of ulcers, but also provided validation of the "rational drug design" approach to R&D. Taagmet was the first of the histamine H2 receptor inhibitors. It was specifically designed to block H2 histamine receptors which were known to affect the process of acid secretion. Within a few years, it had become the largest selling drug worldwide. This drug by itself had a disproportionate effect on the returns for the full portfolio of 1970s new drug introductions. Indeed, when this one drug was removed from the portfolio of 1970-79 drugs, the average present value for the remaining compounds declined by 14%.[2] Tagamet was eventually replaced by another H2 blocker, Zantac, as.

In the past, the usurers were severely penalized. Today, a managerusurer is given a salary of 1 billion dollars a month. Why do people have to pay 5-10-50-100 times more for a drug? There are two reasons: 1 ; patents and 2 ; trade marks. 1 ; A drug prior to its introduction on the market is patented. The owner of a patent has exclusive rights, is a monopolist. He can fix any price, and the people have to pay it. Health insurance helps them. Patent expires after 20 years. 2 ; A drug prior to introduction on the market is given the trade name, the trade mark, the brand name, e.g. Zantac for ranitidine. The trade mark name ; is exclusive. The trade mark is valid for ever. Within 20 years of drug monopoly due to a patent ; doctors and patients get accustomed to the brand name. When after 20 years free production and trade is allowed, competitors are prohibited to use the famous, well known, brand name, e.g. Zantac. They have to call their identical products differently e.g. Ranitidine. But the law prohibits the competitor to say that his ranitidine is an equivalent of Zantac. Surprisingly, it is regarded as a symptom of dishonest competition. It "abuses" one`s property. So doctors continue prescribing expensive Zantac and there is no demand for cheap ranitidine. Nowadays, manufacturing and selling medicines has become almost entirely monopolistic. After patent expiration the competition is illusory. Monopoly persists. First discoveries of indecent exploitation of patents and trade marks in pharmaceutical industries were made in the USA by senators Kefauver and Harris in 1960s. Their revelations were confirmed by auditors in other countries. Remedies and preventive measures were sought. The first measure was a legal provision obliging drug makers to use not registered name ranitidine ; in addition to the brand name e.g. Zantac ; . Shortly after, rich industries created "science" for explaining that a monopoly is not a monopoly, that there is sharp competition in the monopolistic pharma business. Monopolistic Zantac ranitidine ; competes with monopolistic Tagamet cimetidine ; . The maker of cimetidine has to spend much money for convincing doctors that cimetidine is better than ranitidine. The maker of ranitidine does the opposite. Two different products compete with each other. Industrial "scientists" do not wish to notice that real competition, the competition of ranitidine with ranitidine brings the price down from $ 1.90 to $ 0.024 see Annex Table 1 page 4 ; . The competition of the monopolistic ranitidine Zantac ; with the monopolistic cimetidine Tagamet ; raises prices. Industry-friendly scientists invented an argument that a monopolistic drug has to be expensive because of it involves high cost of research and development. There is a plenty of statements that inventing a new drug costs $ 500 billion. Some say - $ 800 billion or more. It is a great humbug. Nobody knows the truth, the industrial data is secret, no company had documented.
