Amundsen; Duke University Medical Center, Durham, NC OBJECTIVE: Nocturnal polyuria has been shown to be increased with age in men complaining of nocturia and overactive bladder symptoms. Biological models have demonstrated a blunting of diurnal arginine vasopressin release over age 60 in both men and women. Our objective was to determine the prevalence of nocturnal polyuria in a group of women complaining of nocturia and overactive bladder OAB ; symptoms. In addition, we sought to determine if the risk of nocturnal polyuria increases at age sixty and over and to describe the impact on quality of life due to these symptoms. METHODS: Women presenting to a urogynecology clinic with complaints of nocturia waking at least twice per night to void ; and OAB symptoms including frequency, urgency, or urge incontinence were asked to participate. Subjects completed a 3-day voiding diary, the Nordic sleep questionnaire, the urinary distress inventory UDI-6 ; , and a nocturia distress visual log. The 24-hour urine production, night time urine production, and functional bladder capacity were determined from the bladder diary. Nocturnal index night time urine production divided by functional bladder capacity ; was calculated. Nocturnal polyuria was defined as production of 33% of urine volume during an eight hour sleep cycle. Subjects were divided into two groups: age 60 and age 60 and the outcomes were analyzed. Subsequently, subjects were redivided into those with and without nocturnal polyuria for further analysis. RESULTS: Fifty-five women participated: Nine subjects were age 60 and forty-six were age 60. Mean values for UDI, nocturnal distress visual log, sleep, and insomnia scores were 42.7 19.2, 7.39 and 12.24 5.37 respectively. There was no significant difference between the age groups in any of these outcomes. Among these groups there was also no difference in 24-hour urine production; however there was a significant difference in the incidence of nocturnal polyuria. Thirty-one 67.4% ; subjects 60 had nocturnal polyuria, while no subject 60 had nocturnal polyuria p 0.001 ; . The rate of nocturnal polyuria increased linearly with age over 60. UDI, nocturnal distress visual log, sleepiness, and insomnia scores did not differ based on presence or absence of nocturnal polyuria; however nocturnal index was higher in subjects with nocturnal polyuria, and this result trended towards significance. CONCLUSION: In women 60 complaining of nocturia and overactive bladder symptoms there is a high prevalence of nocturnal polyuria, and this may be a significant etiology for nocturia in this age group. Women experience a high degree of distress from nocturia regardless of its etiology. A bladder diary remains a useful tool to screen for nocturnal polyuria, and treatment should be tailored accordingly.
