Nontraditional risk factors for cardiovascular disease add significant risk to those with and without disease Nontraditional risk factors for cardiovascular disease, such as certain measures found in the blood, increase cardiovascular risk with each factor present, according to a new study being presented on Monday, June 6, at The Endocrine Society's 87th Annual Meeting in San Diego. This study suggests, say researchers, that these suspected nontraditional risk parameters may be more common than previously thought and their contribution to atherosclerotic disease may be clinically significant. Some of the risk factors for cardiovascular disease are relatively well known and understood. These include elevated LDL "bad" ; cholesterol and the presence of diabetes. These risk factors, however, do not account for all of the likelihood of developing cardiovascular disease. Drs. Elizabeth Koller and Cyrus V. Desouza, of the Omaha VA Med Center and the University of Nebraska Medical Center in Omaha, wanted to explore some of the putative nontraditional risk factors, such as lipoprotein a ; , homocysteine, highly sensitive C-reactive protein hs-CRP ; , and urinary protein in a diabetic, primarily male population. They reviewed the records of a clinic patient population for evidence of cardiovascular disease, such as hardening of the arteries in the heart, brain, and leg blood vessels; lab results for traditional risk factors, such as glucose control and cholesterol level; and lab results for the four nontraditional risk factors in addition to other clinical features. They identified 104 patients who had at least one of the four nontraditional risk factors when assessed during an 18-month period. Approximately 50 percent of the patients had evidence of cardiovascular disease. Abnormal values were found for all four of the nontraditional risk factors. When the numbers of patients with and without nontraditional risk factors and with and without cardiovascular disease were compared, the risk for cardiovascular disease appeared to be greater in patients with any of the nontraditional risk factors, even though the cardiovascular risk was not equal for all factors. Furthermore, the risk from the various factors appears to be additive: Patients with a greater number of abnormal nontraditional risk factor parameters appear to have more risk for disease than do patients with no nontraditional risk factors or only one nontraditional risk factor. The researchers say that the findings need to be explored in larger patient populations and using prospective study designs. Testing for these parameters could become more routine even in patient populations with less cardiovascular disease than in this study, believe researchers. We evaluated whether women with this syndrome had a deficiency in endogenous and induced interferon-alpha production. STUDY DESIGN: Blood was collected in heparinized tubes from 62 women with vulvar vestibulitis and 47 control women of similar age and ethnicity. Whole blood cultures were incubated in the presence of 0.1 ng mL lipopolysaccharide induced ; or culture medium uninduced ; for 18 to 20 hours. Aliquots were tested for interferon-alpha levels by enzymelinked immunosorbent assay. Vestibular samples were tested for human papillomavirus by polymerase chain reaction. Aliquots were also characterized for alleles of the polymorphic gene, interleukin-1 receptor antagonist, by polymerase chain reaction. RESULTS: In uninduced cultures, interferon-gamma was present in 68.1% of control subjects as opposed to 33.9% of vulvar vestibulitis patients P .0005 ; . Similarly, after lipopolysaccharide stimulation, 70.2% of control subjects and only 48.4% of patients produced interferon-alpha P .03 ; . Among the positive samples, however, there were no differences in the interferon-alpha levels between patients and control subjects. In contrast, induction of interferon gamma in response to lipopolysaccharide was similar in control subjects 78.0% ; and vulvar vestibulitis patients 82.1% ; . Women who have a deficiency in interferon-alpha production did not have an increased prevalence of human papillomavirus infection.
