We present an application of a novel methodology called Text Influenced Molecular Indexing TIMI ; to mine the information in the scientific literature. TIMI is an extension of two existing methodologies: 1 ; Latent Semantic Structure Indexing LaSSI ; , a method for calculating chemical similarity using twodimensional topological descriptors, and 2 ; Latent Semantic Indexing LSI ; , a method for generating correlations between textual terms. The singular value decomposition SVD ; of a feature object matrix is the fundamental mathematical operation underlying LSI, LaSSI, and TIMI and is used in the identification of associations between textual and chemical descriptors. We present the results of our studies with a database containing 11 571 PubMed MEDLINE abstracts which show the advantages of merging textual and chemical descriptors over using either text or chemistry alone. Our work demonstrates that searching text-only databases limits retrieved documents to those that explicitly mention compounds by name in the text. Similarly, searching chemistry-only databases can only retrieve those documents that have chemical structures in them. TIMI, however, enables search and retrieval of documents with textual, chemical, and or text- and chemistrybased queries. Thus, the TIMI system offers a powerful new approach to uncovering the contextual scientific knowledge sought by the medical research community and diamicron, for example, locoid.

Safety evaluation: Is this target organ toxicity related to the pharmacology? What is the mode of action MOA ; ? Is the target organ toxicity relevant to humans? Pathology or prior knowledge of the compound class may provide clues on a possible MOA for toxicity. However, if this deductive approach yields no results, the inductive approach offered by new technologies can generate novel research leads. For example, toxicogenomics can generate a gene expression profile of the toxicity which can be compared with that of reference compounds or with other candidate drugs. Similarly, proteomic analysis of the protein profile at the toxic versus the efficacious dose can provide clues on MOA for the toxicity, and may allow differentiation of the pathways of the toxic response from those required for pharmacological activity. Jonathan Tugwood AstraZeneca Pharmaceuticals, Cheshire, UK ; spoke on "Application of Genomics Technology to Assist Drug Safety Assessment." Two candidate pharmaceuticals, with the same pharmacological target, were withdrawn from development because of unexpected liver toxicity in rat and dog. This toxicity comprised hepatocyte fat vacuolation steatosis ; , and focal necrosis. To investigate possible mechanisms of toxicity, rat hepatocyte cultures were treated for various times with toxic and sub-toxic concentrations of Reception at Chateau Meursault the compounds. Cytotoxicity and fat accumulation were measured biochemically, and RNA was prepared for gene profiling studies using Affymetrix GeneChips. Applying toxic concentrations of both compounds resulted in fat accumulation in vitro, recapitulating the in vivo phenotype. The changes in gene expression caused by the compounds included some genes known previously to be involved in hepatic steatosis, namely CYP2E1, aldolase B, and manganese superoxide dismutase. Also, two genes previously unknown to be associated with steatosis were shown to be strongly upregulated by high doses of both compounds after 8 and 24 hours treatment. These genes were identified as best5 and Mob-1 IP-10, which are chemokine responsive. This and other data suggest that there may be involvement of factors such as interferon-gamma and TNF-alpha in the toxicity elicited by these compounds. The final presentation "Toxicogenomics: the Use of Gene Expression Profiling in Mechanistic and Predictive Toxicology" was made by Jonathan Moggs Syngenta Central Toxicology Laboratory, Cheshire, UK ; . Toxicogenomics, the analysis of toxicant-exposed biological materials using gene expression profiling technologies, is beginning to facilitate a more detailed interrogation of the mechanisms through which adverse effects are induced by chemicals and drugs, Pre-dinner reception at Chateau with the future promise of informing hazard identification and risk assessment paradigms. The use of gene expression data in this context is hindered by a lack of information on the relationships between transcriptional events and physiological and pathologcontinued on page 20.

Benchmarks in a continuous process that can vary greatly from one person to another. Nevertheless, the Global Deterioration Scale and other similar instruments have proven to be a reliable diagnostic system to generate clinical evidence toward an outline of key symptoms characterizing seven stages ranging from unimpaired function to very severe cognitive decline 21 ; . Agitation Often Reflects an Underlying Infection or Medical Illness--Above and beyond the general symptomatology, a person with probable AD typically manifests what is commonly referred to as agitation. In the early stages of the disease, agitation accompanies memory loss, thinking problems, personality changes, irritability, anxiety, depression, sleep disturbances, delusions firmly held belief in things that are not real ; , hallucinations seeing, hearing or feeling things that are not there ; , pacing, repetitive and restless movement, general emotional distress, and cursing or threatening language. Agitation often reflects an underlying infection or medical illness, pain or discomfort, including loss of hearing or eyesight. Prescription medications for the treatment of ADassociated or non-AD dementias can cause agitation, especially when multiple medications are used. Agitation may be exacerbated by drug interactions, or by circumstances that worsen the person's ability to think, including moving to an unfamiliar environment or variable caregivers. Agitation can disrupt patient care, and interfere with the ability of the patient or the caregiver to carry out activities of independent daily living. The treatment of agitation depends on a careful diagnosis, determination of the possible causes and the types of agitated behavior the person is experiencing. With proper pharmacological treatment and intervention, significant reduction or stabilization of the symptoms can often be achieved 21, 26, 27 ; . Atypical anti-psychotic and anti-convulsant medications with mood-stabilizing properties are most commonly used to treat agitation 20, 25, 2730 and dramamine.
This has recently been reviewed comprehensively in the Drug and Therapeutics Bulletin 2003; 41 2 ; 12-14. In general, decisions on the exact drug to be used should be made through an Ophthalmologist. As a rule, the topical prostaglandin preparations are used as a second-line treatment. However, in certain circumstances, these preparations are used as first-line treatment. Although the preparations are expensive, they are used only once daily and are extremely effective in reducing the intraocular pressure. As a consequence surgery to reduce the intraocular pressure can often be avoided. In Oxfordshire, g.Latanoprost was used exclusively until the end of 2001. In January 2002 two new topical prostaglandin analogues became available: g.Bimatoprost and g.Travoprost. The latter preparations are slightly more effective than g.Latanoprost in some patients, but tend to cause more ocular side-effects in particular, ocular inflammation ; . As there is no significant difference in the cost of the three preparations, the Eye Hospital has elected to use all three preparations. However, most patients will receive g.Latanoprost, with a number receiving g.Bimatoprost or g.Travoprost. Decisions on which preparation should be used will be made on an individual basis and this decision will be conveyed to the General Practitioners by means of a letter. Costs Latanoprost Bimatoprost Travoprost 2.5ml 3ml 2.5ml, for example, cutivate com.

Tricyclic antidepressants TCAs ; have been used for chronic pain management for decades; however, their potential use as topical analgesics is an area of new research. This article highlights recent findings relating to the mode of action of TCAs and summarizes results of animal testing of these agents as analgesics. A review of existing and emerging information regarding localized peripheral administration of other drugs by topical application is also included. Currently, topical administration of nonsteroidal anti-inflammatory drugs, opioids, capsaicin, local anesthetics, and alpha-adrenoceptor agonists is being used in a variety of clinical states. Preclinical data suggests the potential for drug development of additional topical formulations of a variety of agents. Collectively, this scientific evidence indicates that topical analgesics create an efficacious option for adjuvant drug therapy, with minimal risk of significant systemic absorption and drugdrug interactions. This looks as if jurisdiction of a US court can be easily obtained, but there is an old doctrine that enables US courts to deny their jurisdiction even though they found minimum contacts: and that is if they consider themselves as a non-convenient forum forum non conveniens ; which is a matter of wide discretion22. And this procedural instrument is used to keep off cases too artificially connected to a US forum and you can imagine that forum non conveniens is granted more often in cases with US defendants than in those with US plaintiffs and foreign defendants ; 23. These are the rules of jurisdiction, and they make a certain determination of a competent court nearly unpredictable. And yet there are always attorneys pleading for suit in the US, in particular those who charge their clients fees on an hourly basis: to make inquiries about international jurisdiction, minimum contacts, forum non conveniens is extremely time-consuming and, hence, it pays regardless of success. And even if the prospects of accepting jurisdiction are gloomy, an action may have a certain bargaining value when a settlement out of court is negotiated. The question of which law is to be applied in a transnational dispute more precisely: is it US tort law? often is part of this threatening scenario plaintiff's attorneys try to display in order to make the opponent settle the case in favour of their client. There is talk of American product liability rules extremely favourable to victims, of high damage awards, of US-style punitive damages, of easy evidence and the like.24, for instance, c8tivate cream.

The symptoms of depression vary but the following are particularly common in Parkinson's: sleep disturbance fatigue problems concentrating anxiety stress irritability sadness People who are depressed often have difficulty with short-term memory and concentration that can improve with treatment. We all forget where we put our keys. The time to be worried is when we don't remember what a key is for! People with Parkinson's are especially at risk for anxiety and feelings of panic, particularly when faced with stressful situations, e.g. unfamiliar surroundings or large groups of people. You may need professional help to learn to anticipate and deal with these feelings. In addition, when drug therapy is working too well you may experience surges of energy that result in you doing too much, leaving yourself exhausted with many uncompleted projects. It is important to pace yourself sensibly. Family members may require professional help as they can have difficulty accepting your illness. They may also be frightened and apprehensive about possible changes in lifestyle, modifications to careers, and long hoped for retirement plans.

Fisheries Management and Complexity [Fisheries management, complex, non-linear, management approaches] Conventional fisheries management tends to emphasize precise predictions of the behaviour of resource systems in order to modify management actions according to relatively short-term objectives, usually with the stated goal of optimising specific outputs of the system. It is argued here that due to the complex, non-linear relationships in such systems, conventional management approaches are unlikely to bring our understanding of the system to a level where meaningful management decisions can be made. Instead, management has to rely on information from models based on larger-scale, coarse-grained ecosystem behaviour. The type of knowledge and information required for creating such models differs from the data used for conventional management. Local knowledge contributed by fishers may be used to highlight slower, more predictable variables, which may include information on habitats and other basic biological and physical processes. The challenge is to integrate knowledge from different sources, from scientific to local users, into models that can be used for management of the system. Management objectives need to recognize our inability to predict system responses to management and external drivers like the weather, and focus on maintaining the systems resilience to withstand undesirable change in an uncertain environment. The study was carried out in twenty villages of three districts around Sirindhorn reservoir where they operate the Pla Kaew fishery in order to describe the socioeconomic status, the current fisheries management procedures, including the existing regulation, implementation, and structure and institutional arrangement. It was aimed at investigating the fishers' attitudes toward user involvement and co-management of fisheries. Different attitudes of the different interests towards improvement of the fisheries management were also investigated. There were 196 fishers including 96 Pla Kaew fishers and 96 non-Pla Kaew fishers who were interviewed using structured questionnaires. A PRA approach was used to involve all stakeholders in sharing the information. A feedback discussion among stakeholders was used in order to identify strategies for improving the fisheries management. Overexploitation of fisheries resources was the main problem of the Pla Kaew fishery, which was caused by an increasing number of fishers in the previous years and the.
